A survey - help needed

I am conducting a few minutes short survey on "Patterns
in Userids" for my project. We want to study patterns in
usernames or userids created by people when they register on different
websites, etc. I would be really thankful if you could participate in
this short survey. The link is:

almost all fields are optional.. if u feel uncomfortable with a field please leave it blank

The survey has been approved by Institutional Review Board of UIC.

u need not give the websites(optional) which u have created
just ur userids and how u came to conclusion to use that it.
